Top 5
Classroom Role
These students would take on more leadership roles in class. They would be group leaders for activities and delegated to different roles in which to help lead their peers to excellence.
The students would be challenged with more difficult and in depth material. For example, instead of reviewing facts learned in class and preparing a one-page paper on it they would be required to learn additional facts and prepare a one page paper with a fact not covered in class.
I would plan a "relearning lab". In this activity the top 5 students would work with students that performed in the middle tier in order to assist in facilitating relearning. They would help to explain the material to their fellow students in a way that is more collaborative and better suited to their individual styles of learning.

Students would be continually pushed to do their best and if they show continual and sustained improvement they could possibly be tested for being "gifted and talented".

Students should be made aware that being in the "top 5" is not a guaranteed place. Once other students begin to apply themselves more and more they will be trying to take a "top 5" spot. This will hopefully encourage them to continue to work hard. If it does not, they may find themselves in the "middle 12" .
Middle 12
Classroom Role
These students would be encouraged to strive toward the "Top 5" helper roles. Ideally if the differentiation works you will start having more than 5 people in your "Top 5" group. I feel that the students would benefit greatly by having their motivation to learn being to help their peers.
The students would be challenged with appropriate level work. They would, for the most part, be able to handle all material with very little help however they would be encouraged to continue to improve. They would work closely with the "Top 5" students.
In the "relearning lab" these students would be responsible for relearning any mistakes that they could not answer correctly. This would be their chance to improve upon learned material that they did not understand. They would work closely with "top 5" students.
Improvement
Yes
If the students show continual and sustained improvement they may be rewarded with one of the coveted "top 5" spots.
No
Students will remain in the "middle 12" position with the goal being to try new things to help them improve. Progress will continue to be monitored and charted.
Bottom 5
Classroom Role
These students would be responsible for improving. They would work closely, in small groups, with the instructor directly. It would be their sole purpose to learn as much as quickly as possible so they can improve as much as possible before classroom assessments begin.
Any material presented to the students must be carefully thought out with the idea being to present as much information, and as many key concepts as possible in as simple way as possible. For example, I may utilize things such as brainpop or the magic school bus in order to present the information accurately and in a fun way.
During the "relearning lab" the students would work solely with the teacher, in small groups, with the objective being to use that time to try and improve their abilities as much as possible.
Improvement
Yes
Monitor progress and hopefully move up to "middle 12" status after significant progress has been shown.
No
Students will be given ample opportunity to show improvement. If they are unable to do so repeatedly they will be tested for possible special needs.
