A notification of extra support is sent to the parents of the student. The interventionist submits a green form to the MTSS/RTI Coordinator.

The teachers are asked to complete a pink MTSS/RTI Support Form, as well as supplemental forms to provide the team with more information about the learning needs

When the student is not making adequate progress even with long term interventions in place, the teacher, interventionist, and/or teacher leader complete a yellow form together
At this point, the Positive Support Team reviews the yellow form and decides whether or not to have a parent meeting
There might be a student who has gone through all the colors of the rainbow, but the teacher still has concerns, the teacher emails the PST about needing more support.
At this point, either a 2nd parent meeting is held and the possibility of SPED testing is discussed.
Child is evaluated, eligibility is decided and an IEP meeting is scheduled.
