Clara, Madelines care giver, hid the fact that she was letting Olly come over and see Maddy when she knew it was against her mothers rules.
Maddy would lie to her mother about her happiness, she didn't want her mom to know that she wanted to go out and have a life because her mother had dedicated her life to taking care of her.

Emotional Pain
After Madeline got sick in Hawaii she felt as if she was holding Olly back from living his life so she ending this with him and became very depressed.
Madeline's mother went through so much emotional guilt when her husband and son died that it made her go crazy and believe that Madeline had a rare disease.
When Madeline discovers that her mother has been lying to her for over 17 years about her disease she feels so betrayed. Her own mother has kept her locked away in her home for her whole life. She has never gotten the chance to experience life and be a real teenager.
Olly went through a lot of hurting when Madeline refused to talk to him after Hawaii, but that didn't stop him from trying . He would message Maddy everyday for three months, but the pain of her not answering was too much so he eventually had to give up.
Love
Madeline and her mom are of course close, but after Madeline realizes her mother has been lying to her she feels betrayed. This still does not stop the fact that she loves her mom.

Of course the two main characters in the book, Maddy and Olly, love each other, they are practically made for each other. They both fall in love with each others minds and show one another how to live life to the fullest.
The love that Olly has for his mom and sister shows through out the story because no matter what he always stood up for them against his dad. It didn't matter how much stronger his dad was then him, he still did whatever he could to protect them.
Places
Olly's roof, the place where he can escape all the yelling and just think
The quite white room with the big windows and fake plants. The room that is suppose to make Madeline feel like she's outside.
Madelines bedroom window, where she waits for Olly to get home so they can see each other while they IM.

New York, the first place Madeline went as soon as she was well enough to travel. She chose this location because that's where Olly had to move after his mom left his abusive father.
