        Replications in regional Lymph Nodes
        
        10-14 days
        LATENT INFECTION
        Virus replicates in Liver & Spleen generating 2* viremia (w/ rash)
    - - May be diff. to distinguish from HSV in:
        
        immunocompromised
        
        neonates
    - - Vaccine:
        
        Varivax: live, attenuated vaccine
        
        Problems:
        
        Direct induction of disease (can't go to school for awhile)
        
        :warning: "BREAKTHROUGH CASES" = disease in vaccinated patients
        
        Can get shingles
  - - - Within 24 h:
        Redness --> Papules --> Vesicles
        
        Unilateral, dermatomal - sharp limits
        
        Opthalmic branch of CN V1 infected (10%) - vision changes
        
        Low grade fever, anorexia
        
        Most common complication
        :warning:Postherpetic neuralgia:
        
        Pain persists for months
        
        More frequent in elderly
    - - Vaccines:
        
        Zostavax: live, attenuated VZV vaccine preparation
        
        older than 50 yo, boosts immunity
        
        Can get infection flare up at injection site
        
        Shingrix: virus protein antigen :heavy_plus_sign: adjuvant
        
        Vaccine of choice :check:
        
        Recommended for all eligible pts including Zostavax pts (might be very painful to receive)
        
        Short supply, imp. of on chemo, etc.
