Rambam on Questioning
When you ask a question to have a better understanding of something, but this will cause rebuking. Sometimes people ask questions just to gain knowledge about something else. However, people can take it another way and see it as rebuke. In another instance someone can purposely rebuke and cause the same outcome.
Bava Metzia 31a
Student to teacher: How to act in the presence of a teacher and when to rebuke them. If it is student to teacher do not rebuke them in a rude tone, but rather nicely, so they understand your concerns.
Should you rebuke
Yes you should rebuke. If a teacher is not teaching, you should tell them. You go to school to learn and if a teacher is not providing you with the education you came for, you need to tell them.

Yes you should rebuke someone if they are doing something wrong. However, not in a mean tone to embrarase them.

Rabbi Meir prayed for the hoodlums to die, but his wife said for the sins to die and not the sinners. This means to not focus on the whole person and change your view of them based on a mistake. Rather than to pray that the bad stuff go away. Just because someone does something bad, does not mean they are a bad person. Everyone makes a mistake now and then.
Yevamoth 65b
Do not rebuke a scoffer, for he will hate you. But reprove a wise man and he will love you. This means to not rebuke someone who is doing something wrong because they already know that they are doing it and also they will not care. They will get angry because someone will be telling them what to do. However, a wise man wants to build off of their mistakes and become better. So contribute criticism will help them.
