Steps of Identifying a Struggling Student and Providing appropriate support and Intervention

Student's work is not up to grade level standard and is struggling to get passing grades on assessments and assignments
Teacher refers student to the weekly SSS (Supporting Students' Success) meeting to discuss the concern with the student's other teachers, counselors and principals to gain better understanding of the students overall performance and devise a plan to support the student
Teachers individually carry out the plan and targeted intervention in their classes for two weeks and meet again to discuss results of intervention
The devised plan seems to be working and the student is performing better. Teachers continue the support bases on the previous plan until they are confident the student is able to access grade level standards with minimal support and guidance
Student is not responding well to the intervention and needs a support system beyond the general education classroom. The student's teachers, counselor, principals and special education teachers will meet to form an SFT (Student Focus Team) of 4 or 5 people including 1 counselor and 1 special education teacher to determine next steps.
Once the SFT decides the student is evaluated and qualifies for Special Education, the student support team (special education department) provides baseline assessments to the student and creates an IEP/ILP. Parents will be fully informed of the decision made and meet with the Student Support Team to discuss the support the student will be receiving. The student will start receiving Tier 3 support by having an educational assistant support in core classes and takes an Academic Resource for individualized lessons.
If the student has a diagnosed disability and is not able to access grade level standards even with Tier 3 support, modifying curriculums will be considered
Teacher works closely with the special education teacher and educational assistant and collaborates in supporting the student
