Identifying Struggling Students
Behavioral
Step 1:
Discuss the problematic behavior with the Chinese homeroom teacher, discover if there are any known behavioral issues and what if any is the school's preferred action, and what previous steps to address the behavior have been successful.
Step 2:
Inform the student's parents of the behavioral problems and the potential solutions. With parental and school permission implement those solutions and periodically reevaluate the issue to see if the solutions have been successful.
Solutions to Behavioral Problems may include: a quiet space for students to collect themselves during periods of stress or anxiety, rearranging classroom seating to avoid conflicts between classmates, rewards for good behavior

Discuss the student's social struggles with other teachers, primarily the Chinese homeroom teacher to discover known issues with the student and find out what step have already been taken to address the problem
Step 2:
Inform the student's parents of the social issues and the potential solutions. With parental and school permission implement those solutions and periodically reevaluate the issue to see if the solutions have been successful.
Solutions to Social Issues may Include: Spending more time with the student to discuss any problems they may be having, pairing the student with a buddy classmate to help with peer interactions

Disruptive Behavior Due to Missing Social Cues
Academic
Step 1:
Discuss the problem with the student directly, find out if the problem lies with comprehension of the English in the lessons, the science itself, or if the student has misplaced their book, or if there is a deeper or more subtle problem at work.
Step 2:
Discuss the problems identified in step 1 with the student's other teachers, coordinate with them to communicate with the student and the student's parents what we believe the issues are and recommend interventions to support the student
Step 3:
Implement the interventions agreed upon by the teachers, student and parents. Reevaluate the students academic progress regularly and modify interventions as needed
Possible interventions include: RTI, additional time for exams, extra tutoring during elective time, and pairing the student with a buddy who is academically successful and can assist them in and out of class

I'd like to add a caveat here as the assignment calls for a plan to use in our current schools and my school provides no special education services, any differentiation or individualized planning and intervention is done solely by the teachers, individually or together in teams.
