Symptoms

Deep voice, impaired growth as a child.
fatigue, lethargy, depression, malaise

Complications
Goiter: Can compress the esophagus and trachea. May compress superior vena cava, leading to distention in eyelids, syncope with coughing, vein distention.
Hypometabolic state: gradual weakness and fatigue, loss of appetite, yet gaining weight, and sensitivity to col.

Myxedematous fluid in the face and extremities which leads to a swollen tongue, hoarse voice, and musculoskeletal pain.
Infants can fail to grow and have difficulty feeding. They can have jaundice and difficulty breathing
Birth defects, untreated hypothyroidism in mother or fetus.

Risk Factors
Primary hypothyroidism: Due to thyroidectomy, radiation treating hyperthyroidism. Can also be caused by drugs containing goitrogenic agents/anti-hyperthyroidism drugs. Iodine deficiency can lead to hypothyroidism (Hashimoto thyroiditis).

Collaborative Care
-
Nutritionist: Adequate diet the includes iodine. Avoid taking medication with too much dietary fiber.
Therapist: Depression is a symptom of hypothyroidism and taking to a therapist, group, or nurse about ways to improve mood. (Guided imagery, relaxation techniques, meditation)
PT: Can talk to patient about exercises that can be less strenuous when they are having musculoskeletal pain.
Labs/Diagnostic Tests
History, physical examination, and lab tests.
Labs: Metabolic panel can show low levels of T4 and high TSH levels can be indicative of primary hypothyroidism.
If antithyroid antibodies are present, the patient may have Hashimoto thyroiditis.
