app that the pt uses to place their information in in prior to thier in person or virtual appoiment;;;;;;;;;;; the app will ask and inform on the the following** The PrEP Coordinator reaches out to patients, evaluates HIV risk and desire to initiate PrEP, counsels and educates patients on the use of PrEP and HIV risk reduction, orders and monitors necessary laboratory tests for PrEP initiation and follow-up (using standing order protocols), educates patients on the self-swabbing for STIs (as needed) and assesses medical insurance coverage for PrEP.
HCP provider ( during the apponiment virtual or in person) would allow the pt to login ( same login as the patient's app account ) , to further provide deciosn support basedon the pt of set criteria ( aka decioson support needs)
the hcp provider softwre tech willl have the evidencebased deciosn support that black need in the tehnology agrorithm
this would ddress the barrier and facilators of healthcare deciosn support eg time contrait ( you wont need that much time to construct)
the app will also ;;;;;;;;;;;;;;;;medication adherence counselling and inquire about potential side effects. The PrEP Coordinator follows up with the patient every 3 months and assesses for adherence, screens for STIs and schedules laboratory visits as indicated by the CDC guidelines ------- Centers for Disease Control and Prevention.
