Story #2: Rabies is Real
The City of Hamilton and the Ministry of Natural Resources and Forestry partnered together
Implemented a rabies prevention and control initiative
Rabies-awareness was the main objective
Educational posters used to inform the residents of the city
Focus groups were used to test efficacy of campaign messages
“Baccoon” and “Skox” images were selected for campaign
Evaluation was based on reachability and level of knowledge among city residents

Story #1: Bloodspot screening

Story #5: PHN Standards in Manitoba
Manitoba made PHN standards and competences (2015)
Goal was to:
Develop tool and resources to support application and achievement of standards of PHNS (long-term)
Communicate consistent and measurable for PHN roles in prenatal, postpartum, and early childhood (short-term)
(NCCMT) model for evidence-informed decision making (EIDM) directed the process of developing and implementing the standards
Data was collected on different items of EIDM
Such as local community health content
Final product of the project resulted in:

Success was attributed to collaboration with regional health authorities and individual PHNS
Collaboration between stakeholders is important

Story #6: Community Health Data Website
Relevant health data is needed for evidence-based decision making in community health planning in BC
Main goal is to make and provide web-based platform
to allow share useable and consistent data for community health planning in BC
A collaboration between the PPH team at the BCCDC developed the community health data website
The objective of the online resources was to help:
school boards and NGOS in evidence-based decision making for community health planning
Evaluation of program assessed the efficacy of three community health data resources and website platform
Through survey and interviews from stakeholders
Collaborative efforts to support evidence-based decision making in community health planning
Should be driven by needs of local communities
