The app could change the behaviour of patients in many ways. More could seek diagnosis for symptoms or they could put off visiting the doctor. They might opt for an option with a greater or smaller value. The way regular doctor's visits are managed could change.
The 'cost' to the health service is not going to be linear with the number of patients visiting doctors.
Reducing the number of patients each doctor sees may only decrease costs marginally if they are still employed full time unless they are paid per patient visit.
Employing fewer doctors or decreasing their working hours because the number of patients reduces to a certain threshold could save more.
The overhead costs for upkeep of buildings / equipment, etc. may not be impacted as much by the number of patients as the impact on salaries.

Key fact - if the app directs people to more appropriate care, allowing for early intervention and a more dynamic medical system, then the app will be a huge advantage whether there are greater or fewer numbers of people visiting their doctors. *Unless the patients receive no care and die.
