Always consider borderline ovarian tumours as a histological diagnosis when undertaking any surgery for ovarian masses. If suspected, referral to gynae-oncology is recommended. 20% of borderline ovarian tumours appear as simple cysts on ultrasound but the majority will have suspicious ultrasound findings.

Menopausal status: Premenopause (1); postmenopause (3)
Ultrasound score: 1 point for each of multilocular cysts, solid areas, metastases, ascites, bilateral lesions
U = 0 (Ultrasound score 0)
U = 1 (Ultrasound score 1)
U = 2 (Ultrasound score 2-5)

Unilocular cysts
Presence of solid components where largest <7mm
Presence of acoustic shadowing
Smooth multilocular tumour largest diameter <10mm
No blood flow on colour Doppler
M Rules
Irregular solid tumour
Presence of ascites
At least 4 papillary structures
Irregular multilocular solid tumour with largest diameter =/>10mm
Prominent blood flow on colour Doppler
