MOA :check:
- Interfere with transpeptidation reaction of bacterial cell wall synthesis
- Cell wall is made up of peptidoglycan: polysaccharide+ peptide
- Polysaccharide contains alternating amino sugars- NAG & NAM
- A 5 amino-acid peptide is linked to NAM sugar. This peptide terminates in D-alanyl-D alanine
- PBP removes terminal alanine in the process of forming a cross-link with nearby peptide
- Cross links confer rigidity to cell wall
Beta lactam antibiotics are structural analogues of this D-alanine substrate and bind to active site of PBP

Inhibits transpeptidation reaction→ Cell Wall Deficient forms are produced
↓
As interior of bacterium is hyperosmotic, CWD forms swell & burst
↓
Bacterial lysis.
- Lytic effect is also d/t derepression of some bacterial autolysins
- Beta lactam antibiotics kill bacteria only when they are actively growing & synthesizing cell wall
- G + bacteria → more susceptible d/t thicker cell wall
- G -ve bacteria→ less susceptible d/t thin cell wall with alternating lipoprotein& peptidoglycan layers
- Safe→ peptidoglycan layer is unique to bacteria
RESISTANCE :check:
Due to one of the 4 mechanisms:
- Inactivation by ß lactamase
- Modification of target PBPs
- Impaired penetration of drug to target PBPs
- Antibiotic efflux
- ß lactamase pdn. is Most Common
