"The Metamorphosis" does not refer to Gregor's physical change, but instead the change in the rest of the family's feelings on Gregor
Gregor's Mother
Gregor's mother's health suffers from Gregor being around, and she doesn't object to the family getting rid of him.
"His mother lay in her chair, with her legs stretched out and
pressed together; her eyes were almost shut from weariness."(Kafka 70)
Taking care of Gregor, especially with her condition, makes it incredibly hard for Gregor's mother to even breathe. She really doesn't feel for her son anymore.
Gregor's mother has much motherly love for her son, but can bear seeing him in his current state.
"Let me go to Gregor. He’s my unlucky son! Don’t you understand that I have to go to him?”(Kafka 51)
Gregor's mother feels bad for him, and she can't bear to see her son in this state.

Gregor's Father
His father ended up becoming furious with Gregor, to the point of physically abusing him.
“Further flight was useless, for his father had decided to bombard him.”(Kafka 51)
This shows how much strife Gregor's new form is putting onto the family, and how it led to Gregor's dad snapping and physically abusing him.
