According the the CDC around 30,000 cases of Lyme Disease are reported each year.
Lyme disease is most commonly noted in areas liek the northeastern portion of the USA, as stated here by the CDC, “Cases are concentrated in the Northeast and upper Midwest, with 14 states accounting for over 96% of cases” (CDC p.6).

As stated by this chart the two ages most likely to get Lyme Disease are young children and old adults. This is because many children are outside for long periods of time playing, increasing the chances of catching Lyme Disease. Older adults have such a high chance due to their weakened immune systems not being able to fight off the infection right away.
Manifestations
Early localized Lyme disease. This stage normally occurs the first week to month after the bite of a tic. Some common symptoms include lack of eneergy, swollen lymp nodes, adn feeling like the flu. However, no symptoms is very likely during this stage.
Early disseminated infection. This stage is from the range from 1-4 months. A tgis point of the infection its impoerative that treatment begins as it has begun spreading to systems like the cadiovascular and neurological. Some common symtpms taht haopen during this period include an expanding rash around teh bite, pain and weakness in the joints and bones, poor memory, rapid heartrate(tachycardia), and arthiris like symptoms.
Late persistent Lyme disease. This is the most serious stage of the disease and must be treated asap. Some symptoms occuring during this stage include athritis, severe fatique, heart problems, feelings of dizzyness, loss of memory.

Diagnosis
The Enzyme-Linked Immunosorbent assay (ELISA) test. This test cans the patients blood in search for the antibodies against B. burgdorferi. Some downsides to this test is it can show false positives and isn't reliable in the early stages of Lyme disease
Western Blot test. This test if preformed after a positive ELISA test to ensure its findings are correct. This test looks for antibodies of the proteins produced by B. burgdorferi

While looking at the symptoms of Lyme Disease can be somewhat helpful, but any of the symptoms associated with Lyme disease are very common in other diseases like rheumatoid arthritis and Lupus.
Treatment
For the vast majority of patients with Lyme Disease using antibiotics such as doxycycline and amoclcillin have proven to work in a couple of weeks.

Other medications to treat symptoms like painful joints may be prescribed via recommendation of the doctor/PCP.
