Pathophysiology
-
Release of inflammatory mediators (pro-inflammatory cytokines) lead to migration and accumulation of neutrophils which release toxic mediators and can cause uncontrolled coagulation (excess clotting or absence of appropriate clotting)
Increase in vascular permeability (from inflammatory response -toxic mediators) leads to pulmonary edema which reduces surfactant and leads to alveolar collapse - compromising gas exchange.
Fibroblasts and collagen infiltrate, stiffening the lungs. This further reduces gas exchange and leads to an increase in work of breathing
Hypoxia induced vasoconstriction can cause pulmonary HTN, which can lead to RHF
Alveolar closing pressure becomes greater than pulmonary pressure and alveoli collapse (atelectasis)
