A Nurse may assess data pertaining to the pattern of food and fluid consumption of the patient. This can help identify perceived problems associated with the patient's eating habits and can simply be done by obtaining a food diary.
Nurses observations and skin assessments may play a vital role when assessing nutritional patterns. They may look for edema, dryness of the skin, they may obtain weight to determine BMI, and may also determine wound healing ability through this intervention. Labs can also be included by obtaining fasting glucose tests in order to come up with the best plan of action for the patient.
Nutritional patterns focus on educational needs. Promoting activities that positively impact a patient may extend their health-promoting behaviors. For example, adherence to a healthy balanced diet can lead to less hypoglycemic episodes. With this said, a Latino/Hispanic American may be encouraged to drink enough water and have small frequent meals to avoid hypoglycemic attacks. With this positively impacting the patient, they would be more apt to continuing these actions.
Nurses assess the parameters of an individuals exercise, this includes the type, frequency, duration, and intensity of their regular exercises. They also assess how much value an individual places on exercise to help understand their personal feelings toward it. A 24 hour recall of activity can be a useful tool to determine the amount of exercise/activity a patient is getting each day.
A nurses direct observation is an important technique to evaluate adequate performance of self care, activities. These activities include, dressing, cooking, and insulin administration. This technique quantifies the individuals ability or disability to perform activities of daily living.
Activity- exercise patterns provide effective indicators for commitment to health promotion and prevention. The impact exercise has on health status has been documented with increased public awareness. The Nurses assessments, plans and interventions to prevent obesity (one of the biggest risk factors for type 2 diabetes mellitus) have had a huge impact on overall global health and health promotion.
Nurses assess a patients movement capacity, activity tolerance, self-care abilities, use of assistive devices, changes in pattern, satisfaction with activity and exercise patterns and perceived problems.
"The home, neighborhood, and community constitute the family environment." Assessing the environment consists of many factors including home type and conditions, number or members in the size of home, income, and adequate provisions.

Assess family community. What chemicals are in home-is the home equipped with a working fire alarm, CO2 detector? Are there chemical plants nearby, recent spills etc in community? Any known toxic waste issues in community. As a nurse, we should assess these factors and do screening as needed for any toxicity that may occur.
Assessing if the patient has adequate housing, if they are living in a clean environment, free of bugs and rodents. If there is enough space for everyone that is living there and do they have access to enough clean water is an important thing to assess in a patient when talking about their environmental factors.
The nurse can assess if the family have enough provisions of hygiene products, clean cooking area and utensils. What the crime in the area is like? How violent of a community is i-burgleries? Reliable transportation to/from provider appointments? What factors are obstructive to obtaining health care or receiving enough provisions for daily living standards? What resources as a nurse can be provided to assist in obtaining and maintaining necessary provisions.
