If you're repeatedly failing, make it easier! Don't get in the habit of failing. Meet yourself where you are. Small attainable goals --> success --> build slowly!
Choose a small, clear, straightforward task (or decide to work on a task for a short period e.g. 5 min) --> do it --> STOP, take a deliberate break, celebrate victory --> repeat.

Visualize yourself succeeding, what that will feel like, and all the benefits you will enjoy afterwards.
Do you (secretly?) think it is hopeless, stupid, not worth it to try, or out of your control? --> reflect on this. Employ positive thinking/CBT. What's the bright side? What are the good things?

Set goals and do experiments to see how much you can accomplish in 25 minutes, with 5 minute breaks in between. Keep score of how many times you succeed or fail to reach your goal in the alloted time. It's a game of you vs. you!
Anxiety
Reflect on what beliefs are at the source of your anxiety. Identify associated cognitive distortions. Create affirmations that counteract these, and review them frequently. (i.e. CBT triple column technique - negative thoughts, cognitive distortions, alternate thought)
Meditate or do some sort of relaxation/mindfulness exercise. (This could range from prolonged formal mediation to just taking a few deep breaths.)

Break down large, daunting tasks into clear, small chunks. (Keep breaking them down into smaller and clearer bits as necessary until it seems manageable!)
