MAJOR DEPRESSION DISORDER 8/20/2020 ARG
PATHOPHYSIOLOGY
It is a depressive disorder under mood disorders
Characterized by disturbances in the regular mood, behavior, and affect
Also called: clinical depression
Causes impairment in daily life
MEDICATION
SSRIs include citalopram (Celexa), escitalopram (Lexapro), fluoxetine (Prozac), paroxetine (Paxil, Pexeva), sertraline (Zoloft) and vilazodone (Viibryd)
SNRIs include duloxetine (Cymbalta), venlafaxine (Effexor XR), desvenlafaxine (Pristiq, Khedezla) and levomilnacipran (Fetzima)
Tricyclic: imipramine (Tofranil), nortriptyline (Pamelor), amitriptyline, doxepin, trimipramine (Surmontil), desipramine (Norpramin) and protriptyline (Vivactil)
NURSING INTERVENTIONS
Check for medications needed.
Check for need of hospitalization
Provide a safe environment
Identify the level of suicide precautions needed
Contact the family, arrange for crisis counseling
If, hospitalized, follow unit protocols
Identify risk factor
Give patient opportunities to make decision about hospitalization
BEHAVIORAL INTERVENTIONS
Encourage clients to express feelings (anger, sadness, guilt)
A patient with a high-risk will require a constant supervision
Alternative ways to handle feelings of anger and frustration
Use DSM-5 for depression and diagnostic of mental disorders
Identify type of depression
IMPORTANT ASSESSMENTS
Assess for previous suicide attempts.
Assess for Abuse any substances
Do they have peers/friends?
Assess for any suicide plan
Assess for the mental status
Physical assessment
PATIENT EDUCATION
Come up with a plan
Educate patient about depression
Teach relaxation techniques
Teach techniques to reduce stress
Refer patient to a individual or group therapy
