What is Gerd?
GERD is when the stomach acid flows back into the esophagus (Gastroesophageal reflux disease, 2018). This back wash ( acid reflux) can irritate the lining of your esophagus leaving an individual feeling discomfort (Gastroesophageal reflux disease, 2018)

What does this all mean?
Individuals who experience GERD and Barrett's esophagus will have irritation in the throat and will experience heartburn and acid reflux. These symptoms will make eating extremely painful and sometimes medications over the counter will not relieve symptoms.Occupational therapy can assist with education on lifestyle modifications and aid with resources that will help with increasing quality of life.

Positioning
Positioning is extremely important when experiencing GERD. Educating the pt on sitting upright until the food is fully digested might assist with prevention in heart burn and discomfort.

Food Log
Pt will write down what type of foods she is eating to better assist with what type of foods make her symptoms worse. This will help her be aware of the type of foods so she can avoid the foods that make her feel discomfort.

Medication Managment
Assist with a daily routine of taking medication. This could either be the medication containers that have the letter of each day set by her bed. Or a visual schedule to remind her when she should take her medications.

Swallowing Exercises
Swallowing exercises will be implemented to assist with maintain strength in her esophagus as well as assist with swallowing when eating.
Grade up: Increase the amount of time while exercising and use water with exercise. Grade Down:Less time and no water

GOALS
STG
Pt will demonstrate the ability to verbally recite lifestyle modifications such as positiong that was discussed in therapy within 3 days. to assist with decreasing GERD symtpoms
PT/ caregiver will verbalize the understanding of filling out food log within 2 days to assist with decreasing symptoms of GERD and help with feeding/meal prep.
Pt will verbalize the understanding of dietary restrictions while meal prepping to assist with decreasing symptoms of GERD within 2 days.
LTG
Pt will demonstrate lifestyle modifications such as positiong in bed in therapy independently to decrease GERD symtoms within 1 week
Pt/Caregiver will demonstrate the ability to fill out food log with minimal verbal cues within 1 week to assist with decreasing symptoms of GER and help with feeding/meal prep.
Pt will demonstrate meal prep and the understanding of dietary restrictions to assist with the decrease of GERD symptoms within 5 days.
Problem List Clients list includes, decrease in social participation, disturbed sleep , swallowing and eating has been a challenge due to the symptoms. She is struggling with health management and maintenance and she has been eating too big of meal portions.
Assessments
Observation
Observation of how she prepares foods, eating will be one of the most beneficial assessments that will provide a client centered treatment .
Interviewing
After reviewing the pt's chart I would formulate questions pertaining to her lifestyle at home, food history, what she is having the most difficulty with and what she would like to work on while she is in therapy.
Client StrengthsDue to the worsening of the clients she is eager to gain knowledge on how to manage these symptoms.
