4 specific occupation based problems w/ 3 general occupations represented & 2 interventions (8 total)
Barrett requires max assist to scoop food onto spoon due to decreased AROM in both wrist and fingers and cortical visual impairment
Within in the home during mealtime, make sure environment is clear of clutter or other distracting objects. Use a white plate/bowl to provide high contrast so Barrett can differentiate between food and plate/bowl.
-
Barrett requires pureed or soft foods for feeding due to poor postural control of head and neck and low tonicity of facial muscles
Within the home,30 minutes of Oral sensoriomotor stimulation before mealtime

Dynamic Systems Theory
Functional movement occurs when multidimensional systems work in coordination. If a child has limited ROM in their upper extremities this will decrease their ability to create coordinated complex movements to accomplish a task (Case-Smith & Clifford O’Brien, 2015, p. 794).
Neurodevelopment Theory
Children with CP often have challenges with postural control and movement against gravity (Case-Smith & Clifford O’Brien, 2015, p. 794). Movements are influence by contextual factors. These factors help in deciding if the movement will be complex or simple.
Sensory Integration
Children with CP can experience poor sensory processing, in regards to visual, vestibular and proprioception (Case-Smith & Clifford O’Brien, 2015, p. 795).
Effectively described how ONE activity from EACH occupation-based problem would be graded with rationale provided.
Built up spoon handle
Grade up: Decrease size of spoon handle
Grade down: decrease size of spoon handle / decrease weight of spoon
Colored Blocks
Grade up: use smaller blocks or start with all stuck together
Grade down: use minimal HOH assist to help stack blocks or take apart
-
Grade up:involve mom or dad in play, ask Barrett who the ball or balloon should go to next
Grade down: brother sits close and rolls ball directly to him

Barrett utilized the head rest on his wheelchair to keep head upright. Head rest may be too far back, as his visual field was towards the ceiling and not right in front of him.
Barrett spoke few words, many difficult to understand. He was able to understand commands.
