J. Clayton Lafferty; claims to be the most widely used and researched culture assessment tool
Pro: It's an older survey that has been well-used and tested across many industries. Also, the developers are classically trained in organizational behavior.
Con: it's a proprietary survey owned by Human Synergistics, Inc and not especially easy to obtain without paying
Purpose
"Provides an assessment of the operating culture in terms of the behaviors that members believe are required to “fit in and meet expectations” within their organization."
The stated purpose implies that the tool is measuring subjective reports and making it objective - it can be psychometrically problematic

Would you recommend this survey for use in your organization?
No. Although it's a complete survey that has been time-tested it doesn't feel like a good fit for my The Ohio State University Medical Center. It has sound psychometric properties and the results appear to be useful, however it really pigeonholes the organization as a whole and I believe major medical centers are to large and complex to be assessed with this instrument. Additionally, I think that the length would be a deterrent to completion, resulting in low capture percentage. It might be a good tool to assess culture within departments (individual clinics or units) that are smaller and more homogeneous.
