Not everyone is treated the same; those that we have an intimate or personal relationship with are given priority when making moral decisions (this is seen, for example, in the trolley problem when the one person is swapped out with the mom, making the decision much more personal and therefore, that much more difficult)
Family, friends, acquaintances are valued more than strangers
-
...is an activity

"More basically, then, caring may be defined as a form of reproductive labor that fulfills three aims necessary for individual survival, development, and social reproduction." (p.51)
"'...the innate equipment of individuals that is the necessary basis for developing the more advanced capabilities, and a ground of moral concern...'" (p.52) These basic activities must be fulfilled before any advancement or growth can be achieved; these are the building blocks to survive in the world
The activity aims of people are subjective to their location, economic situation, education, and the level of development achieved. Western people have different ideas of necessary caring activities because their most basic needs have been met. People in developing countries still desire and require things like clean water and sufficient amounts of food in order to sustain themselves and their families/villages/countries.
