Differentiation from tamponade: hyperresonant to percussion, absent breath sounds
Precipitating factors
Needle or instrumentation near the neck, chest wall or down tracheobronchial tree

Management

Insert pleural drain, observing the bottle for bubbling and/or swinging (5th ICS, anterior to MAL)
If problem persists, consider Cardiac Tamponade
