Nerves of Gluteal and posterior thigh region
Clunial
Middle
Origin: Lateral cutaneous branches of posterior rami of S1-S3 SN
Course: Exit through posterior sacral foramina and pass laterally to gluteal region
Distribution: Supply skin over sacrum and buttocks
Inferior
Origin: Posterior cutaneous nerve of thigh (anterior rami of S2-S3 SN)
Course: From inferior border of gluteus maximus and ascends superficially.
Distribution: Supplies skin of inferior half of buttocks.
Superior
Origin: Lateral cutaneous branches of posterior rami L1-L3 SN
Course: Pass inferolaterally across iliac crest
Distribution: Supply skin of superior buttocks
Sciatic
Origin: Sacral plexus (anterior rami L4-S3)
Course: Enters gluteal region via greater sciatic foramen; descends posteriorly towards biceps femoris; bifurcates into common fibular and tibial nerves.
Distribution: Supplies no muscles; bifurcation into tibial and common fibular nerves.
Posterior cutaneous
Origin: Sacral plexus ( anterior and posterior division of anterior rami S1-S3)
Course: Enters gluteal region via greater sciatic foramen inferior to piriformis.
Distribution: Supplies cutaneous innervation to the inferior half of buttocks, skin over popliteal fossa and posterior thigh and skin over lateral perineum and supper medial thigh.
Superior gluteal
Origin: Sacral plexus (posterior division of L4-S1)
Course: Enters gluteal region via greater sciatic foramen superior to piriformis; courses b/w gluteus medius and minimus.
Distribution: Innervates gluteus medius and minimus and tensor fasciae latae.
Inferior gluteal
Origin: Sacral plexus (posterior division of L5-S2)
Course: Enters gluteal region via greater sciatic foramen. Divides into several branhes.
Distribution: Gluteus maximus
Nerve to quadratus femoris
Origin: Sacral plexus (anterior rami of L4-S1)
Course: Enters gluteal region via greater sciatic foreman inferior to piriformis, deep to sciatic nerve.
Distribution: Innervates the hip joint, inferior gemellus. and quadratus femoris
Pudendal
Origin: Sacral plexus (anterior division of anterior rami of S2-S4)
Course: Exits pelvis via the greater sciatic foramen; descends posterior to sacrospinous ligamen.
Distribution: Supplies structures in gluteal region or posterior thigh
Nerve to obturator internus
Origin: Sacral plexus
Course: Exits pelvis via greater sciatic foramen; descends posteriorly towards the sacrospinous ligament
Distribution: Supplies obturator internus and superior gemellus
