- - - - PATIENT is ready to take action very soon. Plans on how to make changes to lifestyle or behaviour are being formulated and commitments being initiated. Small tentative steps are being taken such as speaking about their intensions which demonstrates a more positive attitude to change
        
        NURSE can help form action plan to help change the behaviour This can include providing helpful information and establishing links with support groups
    - - PATIENT should be making changes to their behaviours these may be small steps but are positive ones They should be looking at ways to strengthen their resolve to change and fight any urges to relapse This is a difficult stage and patients need to work hard
      - NURSE Should help by reviewing the action plan, providing positive reinforcement of success to date, encouraging maintaining links with support groups and avoiding people and situations that tempt them to behave in an unhealthy way
    - - PATIENT is now contemplating that change is needed They are becoming aware of the positives that can be achieved HOWEVER There may be some ambivalence about changing which can cause them to keep [putting off taking action. There is a desire for change
        
        NURSE can explore the advantages of the change also acknowledge any disadvantages and challenges
    - - PATIENT at this stage they have changed their behaviour more than 6 months ago It is important for people at this stage to be aware of situations that may tempt them to slip back into doing the unhealthy behaviour- particularly in stressful situations
      - NURSE The nurses role at this stage is to minimise the risk of relapse by evaluating success and developing coping strategies
    - - NURSE must provide the patient with relevant information in appropriate format and follow up with discussions
        
        PATIENT Is not contemplating or is unaware that change is needed. They are learning that behaviour must change and are encouraged to think about the positives that can be achieved through change There may be lack of motivation or resistance

    - - PHYSICAL NURSE REVIEW Home, Workplace, School, Neighbourhood, Community SOCIAL RELATIONSHIPS NURSE REVIEW Family, Friends, Intimate Others, Work Colleagues COMMUNITY NURSE REVIEW Adequate income, Employment, Health and Education Programmes, Community Events and Activities
