Chronic Kidney Disease (CLINICAL PRESENTATION (Hematologic and Immunologic…

- - - - NSAIDs: may injure kidney in many ways: direct toxicity leading to acute tubular necrosis (esp w/ conditions like dehydration, CHF,..), induction of [allergic] interstitial nephritis, and development of urinary crystals that obstruct the kidney.
      - Iodinated radiocontrast agents: --> acute worsening of
        kidney function; Pxs who are at high risk for contrast-induced kidney injury [like diabetes] should receive adequate hydration, and V [dose] of contrast agent should be minimized; CT angio more dangerous than angio; The MRI contrast agent gadolinium has been a. w/ severe fibrotic skin condition of nephrogenic systemic fibrosis in patients w/ advanced CKD. در GFR<15 توصیه میشود گادولینیوم تزریق نشود) unless there is no alternative appropriate test
      - Metformin:
        داروی بسیارخوب که کنترل وزن خوبی می آورد. بیمارانی که CKD میشوند و به حدی میرسند که باید metformin را قطع کنیم و روی انسولین میروند--> اضافه وزن--> بار کاری زیادتری بر کلیه وارد--> سریع تر شدن پیشرفت CKD; ) به زبان دیگر، متفورمین خوبه! مگه خلافش ثابت بشه!((. Metformin میتواند در دوز بالا در بیمار با GFR بسیار پایین ، اسیدوز لاکتیک بدهد. در فرد مشکوک با چک کردن لاکتات، VBG و بررسی اسیدوز به نتیجه میرسند. قبلا توصیه میشد اگر کراتینین زیر 1.5 در آقایان و زیر 1.4 در خانمها داشتیم ، مصرف metformin قطع شود اما طبق یافته های اخیر بروز مشکلات ناشی از مصرف متفورمین نادر است و ریسک بسیار پایین می باشد.اکنون توصیه شده در GFR حدود 30 - 44 با احتیاط مصرف شود و اگر GFR < 30 بود متفورمین قطع شود
    - - target BP<140/90 mm Hg in px w/ diabetes or kidney disease; In CKD px w/ diabetes or proteinuria >1 g per 24 h, BP<130/80 mmHg
      - ACE inhibitors or ARBs beyond control of HTN, in Px w/ proteinuria
      - In DM: hemoglobin A1c < 7% + treat metabolic acidosis
  - - - Types
        
        Peritoneal Dialysis
        
        FORMS
        
        continuous ambulatory peritoneal dialysis (CAPD): dialysate manually infused into peritoneal cavity + exchanged 3-5x during the day; nighttime dwell frequently instilled at bedtime and remains in peritoneal cavity through the night
        
        continuous cyclic peritoneal dialysis (CCPD): exchanges performed in an automated fashion, usually at night
        
        ACCESS TO THE PERITONEAL CAVITY
        
        peritoneal equilibrium test: evaluation [of dialysis adequacy and] of peritoneal membrane characteristics that measures transfer rates of urea (D/P urea), creatinine, Na, Glc across peritoneal membrane; Pxs classified as low, low–average, high–average, and high transporters.
        
        Slower (low and low–average) transporters: less complete equilibration, small effective SA, low permeability tend to do well w/ fewer exchanges; Larger volumes allow for greater solute clearance, particularly w/ CAPD; Low D/P Ur, Cr, Na; High ultrafiltration, D/D0 G
        در این افراد زمان هر دیالیز بیشتر و تعداد دفعات کمتر است
        
        rapid equilibration (high transporters): Large effective SA, high permeability, tend to absorb more Glc + lose efficiency of ultrafiltration w/ long daytime dwells; tend to lose larger quantities of albumin + other prs across peritoneal membrane; require more frequent, shorter dwell time exchanges, nearly always obligating use of a cycler; High D/P Ur, Cr, Na; Low ultrafiltration, D/D0 G
        
        در این روش 2 لیتر از محلول دکستروز 2.5 % را وارد فضای پریتوئن می کنند. اوره و Cr و Na را در ساعت 0 و 2 و 4 اندازه می گیرند. حجم مایعی که در جلسه دیالیز برداشته می شود را هم اندازه می گیرند. سپس میزان مواد پایین را در dialysate و پلاسما میسنجند.
        
        optimal dose?
        higher rates of urea + creatinine clearance a. w/ lower mortality rates; Pxs do well when they retain residual kidney function. The rates of technique failure incr w/ yrs on dialysis + correlate w/ loss of residual function to a greater extent than loss of peritoneal membrane capacity; For some: a hybrid approach
        
        Fluid removal enhanced by:
        
        1. Maximizing the osmotic gradient
        
        b. Shorter duration dwells (e.g., APD)
        زمان کوتاه --> تعادل برقرار نشده و گرادیان زیاد
        
        c. Higher dwell volumes
        وظیفه حفظ گرادیان است. در ضمن اگر حجم خیلی زیاد شود، اثر معکوس بر گرادیان دارد.
        
        a. Higher tonicity dwells (e.g., 4.25% dextrose)
        
        2. An osmotic agent w/ a higher reflection coefficient (icodextrin مثل )
        
        3. Increasing urine output (e.g., with diuretics) (if they have residual renal function (urinary clearance of urea and creatinine)) نکته: چه در مریض HD و چه در مریض PD در صورت وجود حجم حتی مختصر ادرار، لازیکس با دوز بالا تجویز می کنیم که به حفظ وضعیت حجمی آنها کمک کند.
        
        COMPLICATIONS
        
        Nonperitonitis catheter-associated infections (tunnel infections): Some managed w/ local antibiotic, others are severe enough to require parenteral antibiotic therapy and catheter removal
        
        weight gain and other metabolic disturbances: Albumin + other prs can be lost across peritoneal membrane--> Hypoproteinemia; dextrose--> hyperglycemia--> hypertriglyceridemia;
        
        Peritonitis: develops when there has been a break in sterile technique; defined by an elevated peritoneal fluid leukocyte count (100/μL, of which at least 50% are PMN); clinical presentation: pain and cloudy dialysate, often with fever; most common culprit organisms: G+ cocci (Staphylococcus), G- rod infections less common; fungal and mycobacterial seen in selected patients; Most cases managed either w/ intraperitoneal or oral antibiotics; when due to hydrophilic G- rods (Pseudomonas) or yeast, antimicrobial therapy is not sufficient, and catheter removal is required
        
        residual uremia
        
        1.5–3 L of a dextrose-containing solution infused into peritoneal cavity + allowed to dwell for u. 2–4 h;
        clearance of solutes + water depends on balance between movement of solute and water into peritoneal cavity versus absorption from peritoneal cavity (absorption occurs across peritoneal membrane into peritoneal capillary circulation + via peritoneal lymphatics into lymphatic circulation); rate of diffusion diminishes w/ time + eventually stops when equilibration between plasma and dialysate reached
        
        PD solutions: hypertonicity of solutions drives solute + fluid removal (dextrose at varying [ ] contributes to hypertonicity): Icodextrin is a nonabsorbable carbohydrate used in place of dextrose (more efficient ultrafiltration) Icodextrin is typically used as “last fill” for patients on CCPD or for longest dwell in patients on CAPD; most common additives to solutions: heparin (prevent obstruction of dialysis catheter lumen w/ fibrin) + antibiotics (during an episode of acute peritonitis) + Insulin in Pxs w/ DM
        
        Contraindications: 1- Unsuitable peritoneum (adhesions, fibrosis, or malignancy) 2- Patients with increased peritoneal membrane transport rate over time(inadequate ultrafiltration); Relative contraindications: Pleuro-peritoneal leakage, Hernias, Significant loin pain, Big polycystic kidneys, Diverticulosis, Colostomy, Blindness, Severe deformant arthritis, Psychosis, Significant decrease of lung functions
        
        Physiology of peritoneal transport
        
        Ultrafiltration ارجحیت دارد→
        
        Hydraulic conductance of peritoneal membrane: Density of small pores and ultrapores + Distribution of capillaries in intrestitium
        
        Reflection coefficient for osmotic agent: RC is between 0-1
        Lower a value, faster osmotic gradient loss (glucose:0.3, icodextrine:1--> icodextrine better);
        در محلول دکستروز وقتی ذرات به غشا می خورد تعدادی برمی گردد. در PD ترجیح این است که ذره وقتی به غشای پریتوئن می خورد خارج نشود و داخل پریتوئن بماند. هرچه این ضریب بیشتر باشد کفایت دیالیز بیشتر است.
        
        Effective peritoneal surface area
        
        Hydrostatic pressure gradient: Normally peritoneal capilary (20 mmHg), intraperitoneal sapce(7mmHg); Decrease in large dwell vol, seated and standing position; بنابراین نباید بیش از دو تا دو و نیم لیتر مایع وارد فضای صفاق کنیم و همچنین توصیه به دیالیز
        در موقعیت خوابیده است.
        
        Oncotic pressure gradient: Opposes ultrafiltration اگر فشار انکوتیک داخل رگ بالا رود مانع از خروج مایع از رگ به پریتوئن می شود و بر خلاف ultrafiltration عمل می کند.
        
        sieving:Solute is held back during water ultrafiltration through ultrapores
        
        Concentration gradient for osmotic agent: Maximal at start of PD
        
        Fluid absorption:
        جذب مایع، برخلاف هدف ما است و اولترافیلتراسیون را کمتر می کند
        Relatively constant rate (1-2ml/min)
        
        Intra peritoneal hydrostatic pressure: Increase amount of fluid absorption; Higher in sitting and standing position; Lowest in supine position
        
        Effectiveness of lymphatics:
        درناژلنفاوی در بعضی ها سریعتر است و زودتر مایع دیالیز را خالی می کنند و برداشت آب و solute کمتر است.
        
        Diffusion
        
        Molecular weight of solute: Lower MW (urea 60) more rapidly transport than higher MW (Cr 113, uric acid 168)
        
        Mass transfer Area Cofficient (MTAC): Combined effect of peritoneal SA, resistance, MW of solute; Analogous of KOA of HD; Diffusive clearance of solute per unit of time in theoretical; Situation, dialysate flow is infinitely high, so max solute gradient
        
        Peritoneal blood flow: No dependency of diffusion to peritoneal blood flow
        جریان خون صفاقی چون خیلی متغیر نیست پس خیلی هم در تغیر میزان انتشار موثر نیست.

        Concentration gradient: Uremic solute + K from cap blood in to peritoneal fluid; Glc, lactate, HCO3 diffuse in opposite direction; Max at start of PD; هرچه تعداد دفعات exchange بیشتر باشد چون اختلاف غلظت را بالاتر نگه می داریم diffusion بیشتر است.; هرچه حجم مایع بیشتر باشد اختلاف غلظت هم بیشتر حفظ می شود و diffusion بیشتر است.
        
        Three- pore model (Transport dependent on surface area of peritoneal capillaries rather than on total peritoneal surface area
        Effective peritoneal surface area: area of peritoneal surface close to peritoneal capillaries to play a role in transport
        
        Small pores (4-6 nm): cleft; transport of small solutes by convection (water also)
        
        Ultrapores (<0.8 nm): Aquaporins in the endothelial cell membrane; transport of water only and sieving
        
        Large pores (20-40 nm): cleft; transport of macromolecules by convection (water also)
        
        Hemodialysis: most common (>90% of patients)
        
        components
        
        dialysate: [k]=0-4 mmol/L; [ca]=1.25-1.75 mmol/L; [Na]=136– 145 mmol/L; [Na] modeling: dialysate [Na] gradually lowered; bc Px exposed to ~120 L of water during each dialysis treatment, water used for dialysate is subjected to filtration, softening, deionization, and, ultimately, reverse osmosis [maye ra ba feshar bala vared mikonand to remove microbiologic contaminants and dissolved ions
        
        Blood delivery system: dialysis machine consists of a blood pump, dialysis solution delivery system, and various safety monitors; blood flow rate: 250–500 mL/min; Negative hydrostatic P on the dialysate side can be manipulated to achieve desirable fluid removal or ultrafiltration; membranes have different ultrafiltration coefficients (i.e., mL removed/min per mmHg); dialysis solution delivery system dilutes the concentrated dialysate w/ water and monitors temperature, conductivity, and flow of dialysate.
        
        Dialyzer: hollow-fiber dialyzer; majority of dialyzers are “biocompatible” synthetic membranes derived from polysulfone or related compounds (versus older cellulose “bioincompatible” membranes that activated the complement cascade).
        
        Dialysis access
        
        Graft: place synthetic substance between artery and vein; quickly available for use; complication: thrombosis of the graft and graft failure, due principally to intimal hyperplasia at anastomosis between graft and recipient vein; In addition to an increased rate of access failure, grafts and (in particular) catheters are associated with much higher rates of infection than fistulas
        
        Catheter: worst access

        Fistula: best access; cephalic vein is anastomosed end-to-side to radial artery; should be placed 6 mths before dialysis for sufficient arterialization; have the highest long-term patency rate of all dialysis; Unfortunately, even when created, arteriovenous fistulas may not mature sufficiently to provide reliable access to the circulation, or they may thrombose early in their development.
        
        “adequacy of dialysis”
        
        clearance: volume of blood cleared of waste products in one dialysis session; depends on
        
        1. blood flow: 300-500ml/min; depends on blood access; more limited in catheter; more in AVF; incr it as we possibly can; clearance incr accordingly
        
        2. Dialysate flow: 500-800 ml/min; incr effective in dialysis quality but not as much as blood flow; when raised to max 800, clearance incr 7-10%
        
        3. Mass transfer area coefficient (KoA): max clearance when blood and dialysate flow considered unlimited
        
        4. Molecular weight (inverse proportionate)
        
        urea reduction ratio (fractional reduction in blood urea nitrogen per hemodialysis session) of >65–70% not a very powerful option equation: 100x(urea before dialysis-urea after dialysis)/urea before dialysis; to calculate an equilibrated URR, collect sample 60 mins after dialysis bc of rebound issue
        
        Efficiency
        بیشتر به KoA بستگی دارد. Efficiency در واقع توانایی یک دیالیزر در برداشت solute هایی با انداره ی کوچک است. ) مثل اوره –سدیم – پتاسیم(. به diffusion و میزان آن بستگی دارد
        
        Flux
        باز هم از خصوصیات غشا است ولی در این جا اندازهی pore ها هم مهم است. هر چه pore ها بزرگتر باشد ultrafiltration coefficient بیشتر است
        High flux dialyser =دیالیزی که water permeability آن بیشتر از ml/hr per mmHg 15-20 باشد .)دیالیزر high flux یعنی سوراخهایش بزرگ هستند!(
        
        Kt/V:
        
         K – dialyzer clearance of urea  t – dialysis time  V – volume of distribution of urea, ~equal to patient's total body water
        
        should be above 1.2 or 1.05, depending on whether urea concentrations are “equilibrated; two phenomenons which differentiate KT/V and eKT/V:
        
        Urea inbound:
        وقتی اوره را از بیمار می گیریم , در ابتدا سریعتر برداشته می شود . چون اول خون در دسترس را بر میدارد و اختلاف غلظت هم بیشتر است و افت SUN (serum urea nitrogen) بیشتر است و این قضیه inbound نام دارد .
        
        Urea rebound:وقتی در دیالیز اوره را بر می داریم , همه ی اوره در خون نیست و به تدریج از بافت ها آزاد می شود .در دیالیز اوره های در دسترس را بر می داریم . وقتی دیالیز قطع می شود , اوره هایی که داخل بافت هاست) خصوصاً داخل بافت ماهیچه ای (است وارد خون می شوند و یک rebound در غلظت اوره ایجاد می کنند که این مساله min 30 - 60 بعد از قطع دیالیز اتفاق می افتد .
        Organ reservoirs of urea : بیشترین مخزن اوره عضله ی اسکلتی است. چون خونرسانی اش قوی است و آب زیادی هم دارد. و کسر زیادی از rebound از عضله اسکلتی حاصل می شود
        
        مزیت: علاوه بر محاسبه ی ,URR زمان دیالیز + مقدار تولید اوره را هم در نظر می گیرد و با در نظر گرفتن ultrafiltration مقدار solute ای که که با این روش برداشته می شود را هم محاسبه می کند . بنابراین معیار دقیق تری برای تعیین کفایت دیالیز است . دو ایراد URR را رفع میکند
        
        used to quantify hemodialysis + peritoneal dialysis treatment adequacy;
        
        12 h required each w, u. divided into 3 equal sessions; “dose” should be individualized, and factors other than the urea nitrogen should be considered, including hyperkalemia, hyperphosphatemia,...
        
        Complications
        
        Hypotension: most common acute complication of hemodialysis; when SBP<90mmhg or SBP fall during dialysis>20-30mmhg
        
        causes:
        
        excessive ultrafiltration with inadequate compensatory vascular filling,
        اگر بین دو جلسه مریض IDWG(inter dialysis weight gain) بالا باشد)نباید بیش از روزی یک لیتر باشد تا مجبور نباشیم بیش از 5 لیتر در ساعت از بیمار حجم بگیریم( ما مجبوریم ultrafiltration را بالا ببریم و در یک ساعت حجم بیشتری از مریض بگیریم. که در این شرایط احتمال hypotension بیشتر است .
        
        impaired vasoactive or autonomic responses, که در مریض های دیابتی یا CKD یا ESRD
        
        osmolar shifts:
        وقتی waste product را از خون می گیریم چون اسمولالیته در بافت بیشتر است
        مایع را از داخل عروق به بافت shift می دهد و یک osmotic shift خواهیم داشت که باعث hypotension می شود
        
        overzealous use of antihypertensive agents,
        
        reduced cardiac reserve
        
        وقتی ایسکمی بافتی رخ می دهد آدنوزین آزاد می شود که آزادسازی نور اپی نفرین را بلاک می کند .
        
        . اگر پتاسیم مایع دیالیز پایین باشد hypotension خواهیم داشت
        
        management:
        
        discontinuing ultrafiltration (transmembrane pressure zero so only diffusion), performance of sequential ultrafiltration followed by dialysis,
        
        administration of isotonic saline or hypertonic saline, or salt-poor albumin.
        
        careful evaluation of the dry weight
        افزایش وزن آنها بین دو جلسه ی دیالیز زیاد نباشد . یعنی dry weight را بالا ببرد.
        وزنی که بیمار نه ادم دارد نه افت فشار و نه هایپوولمیک است.
        
        ultrafiltration modeling: more fluid removed at beginning rather than end of dialysis
        
        cooling of dialysate during dialysis,
        به اندازه 0.5 درجه سرد تر از دمای tympanic membrane یا در محدوده
        دمایی 5 / 35 تا 36 باعث vasoconstriction و بهبود فشار می شود
        
        avoiding heavy meals during dialysis.
        چون خون را مستقیم به سمت اسپلانکنیک می کشاند و فشار را کاهش می دهد
        
        پرهیز از ایسکمی بافتی
        
        استفاده از دارو هایی مثل midodrine (selective alpha1 adrenergic agent) – sertraline – fludrocortisone – vasopressin
        
        کاهش جریان خون عبوری از دستگاه نقش زیادی در درمان hypotension ندارد
        
        در بیماری که همراه ESRD ، آنمی دارد، باید آنمی اصلاح شود.
        
        Muscle cramp
        
        cause: Changes in muscle perfusion bc of excessively rapid volume removal or targeted removal below Px’s estimated dry weight;
        در هایپوکلسمی/هایپومنیزمی/هایپوکالمی muscle cramp داریم.معمولا در ماه اول بیشتر اتفاق می افتد
        
        Strategies used to prevent:
        
        reducing volume removal during dialysis,
        
        ultrafiltration profiling:
        ابتدا UF (ultrafiltration) زیاد است و بعد به تدریج آنرا کم می کنیم
        
        sodium modeling:
        ابتدای دیالیز محلول dialysate سدیم بالاتری دارد و به تدریج سدیم افت می
        کند ایراد آن این است که چون سدیم وارد خون می شود ممکن است weight gain بین دو جلسه دیالیز بیشتر شود
        
        استفاده از محلول های هایپر تونیک )مثل سالین/گلوکز)اگر مریض دیابت ندارد انتخاب بهتری است(/مانیتول
        
        اولین خط درمانی هم stretching exercise ها هست که در پیشگیری و درمان موثر است . این تمرینات در خلاف جهت cramp هستند
        
        K,Mg,Ca مایع دیالیز نباید پایین باشد
        
        دارو هایی مثل بیوتین و کارنی تین و oxazepam و ویتامینE
        
        Compression devices
        
        Nausea and Vomiting
        
        معمولا به دلیل
        
        1 - Gastroparesis
        
        2 - Disequilibrium syndrome
        
        3 - Hypotension
        
        4 - آلودگی مایع دیالیز
        
        5 - واکنش های تایپ A و B دیالیز )حساسیت به محلول دیالیز(
        
        درمان:استفاده از antiemetic در حالت های زمینه ای- پرهیز از hypotension
        
        Headache
        
        علل:
        
        1 - caffeine withdrawal
        
        2 - disequilibrium syndrome : در افراد با مصرف بالا کافئین سردرد های میگرنی
        
        3 - Mg deficiency
        
        4 - علت های نورولوژیک
        
        درمان:استامینوفن/ کاهش سدیم مایع دیالیز/ قهوه قوی/ مکمل منیزیم با احتیاط
        
        disequilibrium syndrome: یک سری علائم سیستمیک و نورولوژیک ایجاد می کند مثل تهوع، استفراغ، سردرد، تشنج ، کما
        
        علت :
        
        1 . اوره و waste product سریع می گرفته--> اسمولاریته مغز بالاتر-->آب به داخل مغز شیفت--> ادم مغزی
        
        2 . تغییرات حاد در PH عه CSF که روی نفوذپذیری عروق مغزی اثر می گذارد.
        
        درمان :
        
        1 - کاهش blood flow در جلسات اول دیالیز ) 180 تا 200 )
        
        2 - کوتاه کردن جلسات دیالیز در جلسات اول چون بیماران در ابتدا اوره زیادی )بالای 100 ( دارند )اگر هم حالتی شدید ایجاد شد دیالیز را قطع می کنیم(
        
        3 - URR: باید کمتر باشد )حدود 40 )%
        
        4 - سدیم مایع دیالیز نباید کم باشد )در صورتی که بیمار هاپیرناترمی داشته باشد 2 - 3 میلی مول کمتر از پلاسما نباید باشد(
        
        Sodium modeling در موارد مزمن
        
        Anaphylactic reactions to the dialyzer, particularly on its first
        use, reported most frequently w/ bioincompatible cellulosic-containing membranes
        
        Type A: IgE mediated intermediate hypersensitivity reaction to ethylene oxide used in sterilization of new dialyzers; typically occurs soon after initiation (within first few mins) and can progress to full-blown anaphylaxis if therapy not promptly discontinued; Treatment w/ steroids / epinephrine may be needed if symptoms severe.
        
        type B: a symptom complex of nonspecific chest and back pain, which appears to result from complement activation and cytokine release; symptoms typically occur several mins into dialysis run and typically resolve over time with continued dialysis.

        اقدامی که باید انجام دهیم:
        Immediate stop of blood pump, clamp of blood line
        
        Air embolism
        
        تظاهرات:
        
        Seated patient: pass in to cerebral venous system
        
        Recumbent patient: enter heart and pass in to lungs
        
        Foam in venous blood line
        
        اقدامی که باید انجام دهیم:
        Clamp blood line, recumbent position on left side and head tilted downward, 100% oxygen, aspiration of air
      - initiated when
        
        when complications of ESRD (presence of hyperkalemia unresponsive to conservative measures, persistent EC volume expansion despite diuretic therapy, acidosis refractory to medical therapy) cannot be controlled medically
        
        patient displays signs of uremia [like pericarditis, neuropathic signs like asterixis], u. when eGFR<10 mL/ minute + there are no apparent reversible causes of kidney failure.
        
        a bleeding diathesis
      - outcomes a. w/ both therapies are similar, and the decision of which modality to select is often based on personal preferences and quality-of-life considerations [and residual renal function].
      - Mechanisms
        
        Diffusion: LMW like urea better than HMW like creatinine
        
        Ultrafiltration: برداشت solute توسط ultrafiltration تقریبا با غلظتی معادل غلظت solute اولیه انجام می شود

        بیشتر در برداشت حجم و solute های بزرگ )مثل بتا 2 میکروگلوبولین( موثر است تا برداشت خالص solute
        در این روش برداشت آب معمولا L/h 0.5 - 1.2 است. هر جلسه دیالیز 4 ساعت است و ما نهایتا 4 لیتر مایع می توانیم از بیمار بگیریم**

        به دلیل اثر بر perfusion کلیه و اختلال auto regulation :شریان وابران که انقباض نسبی داشت در اثر ACEI و ARB متسع میشود که باعث کاهش فشار مویرگ میشود و ممکن است GFR کم شود( .بهترین حالت این است که با دوز پایین شروع کنیم. بعضی بیماران کاهش GFR بیشتر از 25 % در عرض 3 ماه دارند که باید بررسی شوند )نکند دیورتیک داده باشیم،دوز دارو زیاد باشد ویا تنگی شریان کلیه داده باشد( .از بیماران درمورد postural dizziness سوال میپرسیم)بعد تجویز دارو سرگیجه پیدا کردید؟بلند میشوید سرگیجه میگیرید؟(
        مثلا فشار منزل بیمار 80 ساله با دارو ها 110 میلیمتر جیوه شده است که با توجه به سن بیمار و اینکه قبلا فشار خون بالا داشته، این عدد فشار پایینی است.با افزایش اندک فشار این بیمار کراتینین افت پیدا میکند و سرگیجه بهبود میابد. در بیمارانی که درمان ACEI و ARB دریافت میکنند ، باید سطح پتاسیم سرم و eGFR کنترل شود.
