Ischaemia leads to the development of acidosis, reduces ATP formation, loss of the normal ATP Na/K pump, loss of myocardial membrane integrity and release of noxious chemical signalling agents (inc. actin, serotonin, bradykinin and histamine) and stimulate un myelinated nerves in the myocardium

The discomfort of angina is referred to the corresponding dermatomes that supply sympathetic afferents nerves of the same segments of the spinal cord of the heart. This is why you can also feel pain in the chest, neck, lower jaw and down the arm. Dermatomes C5-C6 and T1-T6
