Diagnostic assessments are FOR learning. They give teachers an educated guess of what students already know and influence what is taught for that lesson or unit.
In an elementary school setting, spelling tests are quite common. A teacher can choose to do a pre-test with a specific set of spelling words. From there, the teacher and the students can see how many words they can already spell and what they need to learn.

Depending on the activity, it could feel time consuming.
Formative assessments are assessments OF learning. They are part of the students' learning process and can be altered to fit student needs.
Elementary school students need to constantly be monitored for progress. Formative assessments in the classroom may include quizzes, exit slips, Think-Pair-Share, group discussion, and so on.
HIGH STAKES
Most often designed to be a test that is used to make critical decisions about students, educators, schools, and/or districts

High stakes assessments are assessments OF learning. It many cases, it is the final result for a specific subject or topic.
Every student in elementary school is required to take standardized tests throughout their elementary years. Data is collected, results are shared with the student, and all results are made public. An example of this is the End-of-Grade (EOG) test in North Carolina. The test covers several subjects and is administered over a few days.

Self assessments are assessments FOR learning. It is about a process and does not necessarily show the final result.
In an elementary school setting, self assessment can be used in several ways. It can help younger students reflect on their behavior and how well they follow the rules. For older students, self assessment can lead to more self awareness in group projects.
PORTFOLIO

Advantages
There is versatility in how long to build a portfolio - one unit, one month, one year, etc

Disadvantages

It probably requires from other teachers and educators to ensure grading is unbiased, and that can be time consuming for those people.

Art class in an elementary school setting is a great place to use a portfolio assessment. Students can include specific projects required by the teacher and are free to add their own.
