MYOCARDIAL DISEASE (MYOCARDITIS, CARDIOMYOPATHY)
CARDIOMYOPATHIES - group of diseases that affect heart muscle. injury to the myocardium may occur as a result of a myriad internal or external processes
DILATED CARDIOMYOPATHY
dilatation and systolic dysfunction of one or both ventricles where, in general, the LV is frequently affected in isolation. in addition to chamber dilatation, the ventricular wall is often abnormally thin
AETIOLOGY
KEY CAUSES
-
genetic (25%)
inherited as autosomal dominant: a variety of single gene mutations have been identified. there are also autosomal recessive, x-linked (esp duchenne's and becker's muscular dystrophies), and mitochondrial inheritance patterns
-
infective
-
VIRAL causes: PARVOVIRUS, COXSACKIE A & B, INFLUENZA, ADENOVIRUS, ECHOVIRUS, CMV, HIV
-
-
PATHOPHYSIOLOGY
Enlargement of all 4 chambers may occur, wit hthe LV most commonly affected.

CLINICAL FEATURES
KEY FEATURES
presents with heart failure symptoms: breathlessness, peripheral oedema, fatigue, syncope, sporadic chest pain

-
EXAMINATION FINDINGS
Remember to ask about a history of viral illness or myocarditis in the past and a pos fam hx may be helpful

-
IX
FIRST LINE (same as HF)
-
CXR
features of HF, cardiomegaly
Bloods
including LFTs, electrolytes, TFTs, and ferritin (note that haemochromatosis is an important reversible cause)
