GARDASIL
- Quadrivalent
- 16 + 18 (HPV disease)
- 6 + 11 (warts)
- NOW Gardasil 9 (nonavalent)
- 31, 33, 45, 52 and 58 in addition
CERVARIX
- Bivalent
- 16 + 18 (HPV disease)
EXAMINATION
GENERAL
- Well/unwell
- Body habitus
- Lymphadenopathy
-
ABDOMEN
- Inspect - Scars/lesions
- Palpate - Masses, hepatosplenomegaly, tenderness
VAGINAL
- Speculum - Fungating or other growth, bleeding
- Bimanual - Firm, nodular, etc.; mobility and position of uterus
INVESTIGATIONS1) Bloods
- FBC
- UEC
- LFTs
2) Imaging
- USS pelvis + kidneys
- CXR
- CT and/or MRI
- Cystoscopy
3) Examination under anesthesia with cervical biopsy
COMPLICATIONS FOLLOWING LLETZ/CONE BIOPSY
- Moderate to severe bleeding
- Cervical stenosis causing dysmenorrhea, amenorrhea, subfertility, preterm labour and miscarriage
- More likely with repeated procedures
SPREAD OF CERVICAL CANCER
- Directly to vagina, bladder and rectum
- By the lymphatics (iliac then para-aortic nodes)
- By the bloodstream (liver, lungs, bone)
- Once primary lesion >4 cm, poor survival figures as there is a 40% chance of lymph node involvement
