CHOLINERGIC DRUGS:
NEOSTIGMINE
(Neostigmine injection BP)
METHYLSULFATE
(Neostigmine methylsulfate)
Mechanism of Action:
Reversible cholinesterase inhibitor. At moderate doses does not cross the blood brain barrier.
Indications:
Reverses effects of non depolarising neuromuscular-blocking agents
Treatment of myasthenia gravis (acute exacerbation)
Prevention and treatment of post-operative intestinal atony and urinary retention
Nursing Consideration:
Contraindications:
Hyperthyroidism
Increased tone of parasympathetic nervous system
Coronary occlusion
Sinus bradycardia
Arrhythmias
Increase in secretion of the lungs
Asthma
Peptic ulcer
Peritonitis
Urinary tract obstruction
Seizures
Mega colon
Side Effects:
Nausea, vomiting, abdominal cramps, flatulence
Miosis nystagmus
Headache, restlessness, agitation, slurred speech, drowsiness, dizziness, decreased consciousness
Increased desire to urinate or defecate
Sweating, increased lacrimation, bronchial secretion and salivation
Muscle cramps and fasciculation, weakness, ataxia
Bradycardia, hypotension, syncope
Rash, urticaria
Dyspnoea, bronchospasm, wheezing
(Rare) cardiac arrest, coma, convulsions, paralysis, respiratory depression, allergic reaction
Route of Administration:
Intravenous (IV)
Subcutaneous (SC)
Intramuscular (IM)
Storage Conditions:
It should be kept in a cool, dry place where the temperature stays below 25°C and protected from light.
PYRIDOSTIGMINE BROMINE
(Mestinon)
Mechanism of Action:
Cholinesterase inhibitor. Onset of action 30-45 minutes, duration 3-6 hours.
Indications:
Myasthenia gravis
Nursing Considerations:
Route of Administration:
Orally
Storage Conditions:
The recommended maximum storage temperature for Mestinon tablets is 25°C. They should be protected from light.

Bradycardia
Asthma
Vagotonia
Hyperthyroidism
Arrhythmias
Peptic ulcer
Impaired kidney function
Hypersensitivity
Side Effects:
Nausea, vomiting, abdominal cramps, flatulence
