If concerns persist over time and with multiple teachers, someone is assigned to complete the SST Referral (teacher or counselor).
Students identified through data metrics are brought back to Grade-Level Team meetings for confirmation and action planning.
If the evidence shows students continue struggling for 4-6 weeks despite classroom interventions, initiate SST Referral.
Quick discussions (1-2 mins per student), note-taking on Google Sheets
