- Never stopped to build training program - evolved organically out of methodical approach to evolving our processes
- But standardizing work processes and prepping curriculum was tough
5 Whys

Each time problem - solve the whys and the process - thereby a small incremental investment rather than big training development one

Requirements of 5 whys
- Demands environ of mutual respect and trust
- If not - simplified version: Tolerate every mistake once - don't let it happen again - though just a start
- Someone with significant authority be there to implement - act as refree
starting
- Start with a relatively small and specific prob - so ppl get used to it
- Possible to have a date after which an issue will trigger an automatic meeting (keep it short and specific)
- If too many probs - start with a subset
