E. coli in 70-75% cases and in UPEC serotype O1, O2, O4, O6 and O75 are responsible
2 routes :arrow_forward: Ascending route:- after colonising periurethral area E. coli ascend to reach bladder resulting in cystitis and urethritis & pyelonephritis :arrow_forward: Descending route:- hematogenous seeding of E. coli into kidney resulting in pyelonephritis
Types
Lower UIT :arrow_double_down: Site:- Bladder (Cystitis) & Urethra (Urethritis)
Symptoms:- Local ( Dysuria, urgency & frequency)
Route:- Ascending
occurence:- more common
virulence factor:- Fimbriae
Upper UIT :arrow_double_up: Site:- Kidney & Ureter (pyelonephritis)
Symptoms:- Local & systemic ( Fever, vomiting, abdominal pain)
Route:- Ascending & Descending
occurence:- less common
virulence factor:- Capsular K antigen
Predisposing factor that promotes UTI: :black_small_square: Females:- short urethra and close proximity to anus more chance to getting lower UIT

:black_small_square: Urinary obstruction :- in prostate enlargement & urinary stone
:black_small_square: Pregnancy :- physiological obstruction & Asymptomic bacteriuria
