Carbon Monoxide Poisoning
Etiology
Inhalation of combustion fumes
House fire smoke inhalation
Inhalation of paint removers
Pathophysiology
Inhalation of too much carbon monoxide in the air results in CO replacing oxygen in RBCs
This results in tissues not getting enough oxygen (impedes oxygen delivery), and causes hypoxia
Really deadly in major organs that require tons of oxygen to function
Clinical Signs & Symptoms
Headache and Dizziness
Coma/Death
Blurred Vision
Nausea
Cyanosis
Shortness of Breath and Confusion
Diagnostic Tests
Co-oximeter showing > 15% of HbCO
Blood test for carbon monoxide levels
Normal SpO2 with cyanosis
Differential Diagnosis
Anemia
Cyanide Poisoning
Methemoglobinemia
ARDS
Lactic Acidosis
Respiratory Management
Upon arrival to the ED, if suspected, initiate NRB flush
Hyperbaric chamber
If unresponsive, Intubate for airway protection with conventional settings. 100% FiO2 needed
