[Classification]
Gliomas:
-
-
Astrocytoma
CCC
-commonest glioma
-Mutations: P53, IDH1/2, RB...
-Positive immunostaining for IDH1 (imp) in identifying low grade.
-Ki-67 done for all cases .
-All astrocytomas are GFAP+
-
G1 (Pilocytic)
Composed of :bipolar cells, Rosenthal fibers, eosinophilic granular
Site :cerebellum, 3rd ventricles, optic pathways, spinal cord
-
-
-
G3 (Anaplastic)
-Adult tumor
- Supratentorial but can occur anywhere in the brain
- No **microvascular prolif /palisad necrosis**
- More pleomorphic/cellular
G4 (Glioblastoma)
-Microvascular proliferation and palisading necrosis
-pleomorphic tumor with mitosis
-Supratentorial with edema
-
-
Embryonal (Primitive)
(G4) Medulloblastoma
CCC
-
-
Any midline cerebellar or roof of 4th ventricle tumor in a child is a medulloblastoma until proven otherwise.

Spinal cord tumors:
● Extra/intra-spinal ;metastatic, lymphoma
-
− Extramedullary: schwannoma, meningioma
− Intramedullary: ependymoma, astrocytoma
-
-
note: most common intracranial tumors in adults: Metastatic, Glioblastoma multiforme,
anaplastic astrocytoma meningioma
note: most common intracranial tumors in children are: Astrocytoma, ependymoma, medulloblastoma
multiple sites (Metastasis )
Ring enhancement (e.g edema)
Low grade lesion (defined)
