Signs and Symptoms
chronic generalized myalgia and arthralgia
tenderness in so called „tender points”
chronic fatigue
susceptibility to depression, anxiety,
sleep disturbances
vegetative disturbances
Treatment
- non-pharmacological therapy:
rehabilitation exercises combined with physiotherapy (whole body cryotherapy)
psychotherapy (e.g. behavioural therapy)
- pharmacological:
– SSRIs (fluoxetine, paroxetine, citalopram),
– SNRIs: (Serotonin-norepinephrine reuptake inhibitors, e.g. venlafaxine, duloxetine)
– anticonvulsant: pregabalin, gabapentin
– low dose tricyclic antidepressant (e.g. amitriptyline)
- chronic (>3mo), widespread (axial,left-and right- sided, upper and lower segment), non-articular pain with characteristic tender points
- more in female.
- ages 25-45 yr
- second (after osteoarthrosis) most commonly diagnosed rheumatologic disease
- strong association with psychiatric illness
Polymyalgia Rheumatica
- pain and stiffness of the proximal extremities (girdle area).
- no muscle weakness
- age of onset typically >50 yr,F is more common
Drognosis
-
- blood: anemia, elevated platelets, elevated ESR and CRP, andnormal CK.
Treatment
- goal of therapy: symptom relief.
- prednisone.
