When does it happen?
Can occur anywhere and is due to a breakdown of muscle closure after surgery

TWO TYPES
Sliding hiatus hernia: this is where there is a gastro-oesphageal junction and the stomach. The stomach then slides up through the hiatus and lies above the diaphragm and this means there can be acid reflux and a less competent LOS
Rolling or paraesophageal hiatus: the GO junction remains in the stomach but part of the fundus of the stomach prolapses through the hiatus alongside the oesphagous
