Motor
UMN: CNS motor pathways
- Increase DTR/Muscle Spasticity/Babinski
Damage to C-BIBS:
- Cerebral cortex, Basal Ganglia, Internal capsule, Brainstem, spinal cord
Contralateral:
- Cortex (Focal motor loss),
- Internal Capsule (large one sided motor and sensory,
- Brainstem (Large body deficit, Ipsilateral facial/Head)
Ipsilateral:
- Spinal Cord (motor below certain level)
LMN: Peripheral
- Decrease DTR/Muscle Flaccid/Fasiculations
Anterior horn, Nerve root, Peripheral nerve
Sensory
Contralateral:
- Thalamus (cause pain),
- Cerebral cortex (regular impaired sensory)
- Spinal cord (contralateral pain, temp and touch - 1-2 segments below on contralateral side)
Ipsilateral: Peripheral nerve root
Proprioception
- Metabolic issues: B12, ETOH, B6 OD, viral, chemo, fatigue
- Neurologic disease: MS, Neurosyphilis
- morphogenic change in body: growth, weight, amputation
EXCEPTION: Dorsal columns in spinal cord - Loss inferior and ipsilateral
Consciousness
Bilateral cerebral dysfunction:
Trauma, infections, seizures, metabolic problems, lesions
Trauma: Concussion, Subdural hematoma, subarachnoid hemorrhage, Intracerebral hemorrhage
