1.6 Permit To Work 4/4
Confined Space Entry
An enclosed or partially enclosed space that is at atmospheric pressure during occupancy and is not intended or designed primarily as a place of work, and:
:pen: Is liable at any time to:
:warning: Have an atmosphere which contains potentially harmful levels of contaminants;
:warning: Have an oxygen deficiency or excess; OR
:warning: Cause engulfment; AND
:pen: Could have restricted means for entry and exit
- Monitoring of atmospheric conditions for :explode: oxygen level (20 - 20.9%) :explode: toxic gases level (Permissible exposure Limit) :explode: flammable gases (<10% LEL)
- Positive isolations, lock-out and tag-out
- Draining, purging, steaming of vessel, tank or equipment to be entered
- Ventilation requirements
- Standby man and access control e.g. no-entry sign, entry/exit records
- Medical fitness certificate and training requirements
- Emergency rescue plan and drill

Physical Isolation (PIC)
used for work on process lines and equipment. It states that they have been depressurized and free of any hazardous material, isolated from other equipment in the operating facility and suitable precautions taken to ensure that they remain isolated. Positive isolation is required for confined space entry isolation.

Excavation
Used for any excavation work as defined in PTS Excavation. It specifies the precautions needed to protect underground services e.g. cables or pipes, the workers and the surrounding facilities affected by the excavation
Checking of underground services
Excavation methods i.e. manual or use of machine for depth >150mm
Requirement for shoring, use of ladder and Confined Space Entry certificate (for depth > 1.5 meters)
Radiation
Used when handling ionizing radiation sources. It states that a Radiation Protection Officer (RPO) has granted permission to use the radioactive source and that any related extra precautions have been taken
- Approval and supervision by authorized personnel e.g. Radiation Protection Officer or Supervisor
- Radiation level measurement
- Area classification i.e. Controlled Area, Supervised Area
- Use of personal dosimeter/badge for exposure monitoring
- Control of exposure to general workers, non-essential personnel or public e.g. working during night time/lunch break, barricaded area with warning signs, communication via alarm/siren
Lifting
Applied for lifting activities e.g. using equipment such as mobile crane, crawler crane, and lorry crane. For critical lifts, an additional lifting plan is required
- Proper types of crane (e.g. fixed, overhead, mobile) and certificate of fitness
- Specifications of crane and other lifting appliances e.g. boom length, SWL
- Testing of lifting appliances e.g. slings, shackles
- Crane Inspection Checklist
- Competent and authorised crane drivers and riggers
- Lifting Plan and/or Checklist for critical lifting
Road Obstruction/Closure
Used for controlling closure of roads to ensure that access for emergency vehicles is maintained when section(s) of a road are closed
