General Overview of the liver
Histologically the liver is divided into 1- Lobules. 2- Acini
Portal = biliary
Lobules: hexagonal units,
the central vein in the middle
, and the portal tracts on
the borders
The zonation or acini: it is divided to three zones 1, 2, 3
according to zones depends on the proximity to the central veins or to the biliary triad
any injury especially ischemic injuries the first area to be affected severely is zone 3, because it is already poor oxygenated
Zone 1 being closest to the portal tract is
rich in blood supply whereas zone 3 being the
furthest is poor
Zone 3 = next to the central vein
toxic injury In contrast,
the First zone to be affected would be zone 1 because it is the first to receive the
Blood and so would be the first to receive the toxins
Zone 1 = next to the biliary triad
Contents of
the portal tract
A portal venule (a branch of portal vein)
A hepatic arteriole (a branch of hepatic artery)
The bile duct
Lymphatic vessel
Branch of the vagus nerve
The portal tract is the same as the portal triad
Major clinical syndromes
of liver disease
Hepatic failure
Cirrhosis: due to fibrosis
Portal hypertension
Jaundice & Cholestasis
