Micturition
Nerve supply
-
Hypogastric nerve
Sympathetic sensory and motor to internal sphincter (contracts) and detrusor (relaxes)
If stimulated - Wont urinate
-
-
-
Bladder filling
-
-
When filling sufficient to stretch detrusor--> myogenic reflex = detrusor contracts -> positive feedback
-
-
Bladder emptying
- Activation of parasympathetic motor nerves to bladder (contraction of detrusor)
- High pressure opens internal sphincter passively
- Inhibition of the somatic motor drive to the external sphincter
-
-
-
Nerve damage
PNS
Pelvic nerve damage
Lack of parasympathetic motor neurones to detrusor. Sympathetic supply OK. Bladder will contain urine and have to be manually expressed
-
CNS
Spinal legion
In sacral segment will result in lack of parasymp supply to detrusor and loss of ext sphincter. Sympathetic tone in internal sphincter may be present. Bladder will be full but easy to express
