20 sub classes of descriptive words from which patient has to choose a maximum of one/ class. Further down the list, more points allocated to calculate overall pain rating index.
Description of the pattern of pain from 3 sub-classes of words & then produce some qualitative data about what things relieve/ increase the pain.
A diagram of a body is presented to the patient, to use to mark where the pain is located around the body.
