Nursing Diagnosis
Ineffective Breathing Pattern related to inhaled irritants and allergies as evidence by cough, dyspnea, tachypnea, prolonged expirations, and use of accessory muscles
Goal: Client will maintain optimal breathing pattern as evidence by relaxed breathing , normal respiratory rate or pattern and absence of dyspnea

Client will adhere to medication regimen in order to correct current attack and prevent another one from occurring
Interventions

Nurse will answer any questions that the client or family has regarding the medications. Nurse will educate client on how to administer medication as well as the benefits and side effects of the medications.

Nursing Diagnosis
Anxiety related to the clients perception of a crisis situation of a threat to life as manifested by feeling s of anxiousness and parental figures increasing weariness
Goal: Patient has posture, facial expressions, gestures, and activity levels that reflect a decreased level of distress
Outcomes
Client will verbalize fears, concerns, questions, or feelings of uncertainty.

Client will be familiarized with people, equipment, and necessary procedures
Nursing Interventions
Nurse will encourage and allow client to talk about concerns, or feelings of anxiety as well as ask questions
Nurse will assist client with identifying coping strategies that will enhance the clients confidence
Nurse will familiarize the client with his environment, people, and new experiences
Nurse will assist the client in developing anxiety reducing skills such as deep breathing, relaxation, positive visualization, and reassuring self-statements

Evaluation: Client maintained High-Fowlers position and used pursed lip breathing to obtain a normal breathing pattern. Patient's vital signs were documented WNL after a few days within the PICU. Patient adhered to medication regimen. Nurse informed client on the importance of taking his medication as how to prevent acute asthmatic episodes and symptom management. Nurse and client reviewed triggers to avoid within client's daily life.
Evaluation: Client demonstrated appropriate facial expressions, gestures, and activity levels for his age. Client was able to verbalize his fears and concerns while his mother asked questions. Client was able to identify methods in which he could reduce stress. Client became familiarized with the people surrounding him as well as his environment and began to feel less anxiety. client successfully used to anxiety reducing skills.
