Chapter 20: SCI
-
Classification
-
Paraplegia: complete paralysis of part of trunk and both LE's; results from lesions of T/L SC or cauda equina
-
Complete vs Incomplete
Complete: no sensory/motor function in the lowest sacral segments (S4, S5)
Incomplete: having motor/sensory function below neurological level including function at S4 and S5
Zone of partial preservation: motor/sensory function below neurological level, no function at S4, S5
-
-
-
Active Rehabilitation
-
-
PT interventions
-
-
-
Locomotor Training
Complete SCI
Even if won't be functional ambulators, give option for training - standing itself is beneficial to pt
-
-
-
Incomplete SCI
-
"Train like you walk"
-
-
Posture, trunk, pelvis, and limb kinematics are specific to task of walking
Minimized compensatory strategies, ie. hip hiking, lateral trunk lean
Activity-based UE Training, Health/wellness, Patient Education
-
-
Prognosis
Incomplete lesion (ASIA B, C, or D) greater likelihood of recovery of motor function; even complete lesions (ASIA A), 70% patients with cervical level injuries are likely to experience one level of motor recovery below the original neurological level
