Assistance from the Learning Support Team
Case manager reviews request for assistance, notifies parents, determines appropriateness of case for discussion, dermines if enough level one intervention has been completed, and documents interventions and outcomes.
Case Discussion is held and an Intervention Plan is put into place.
Intervention Plan is reviewed on a 2-week basis and adjusted as needed. Interventions are monitored and data is collected to determine degree of success.
If data does not indicate that target goals are being met, the LST can make a recommendation to for a special education evaluation.
The referring teacher completes the SEN referral form with assistance from the special education teacher and the LST.
All required data and documentation are compiled by the LST (behavior logs, samples of work, attempted strategies, outcomes, test scores etc) and completed referral packet is given to the SEN teacher. A copy is sent to the SEN coordinator for review.
SEN coordinator reviews referral packet for both completeness and appropriate/adequate interventions.

Regular Education Intervention
A teacher, parent, or other concerned party identifies a concern(s) he/she believes is interfering with a student's learning potential. Those concerns are conveyed to the classroom teacher and discussed with the parent.
Teacher review cumulative records and checks with previous teacher(s) to gather data about the student's academic performance and/or behavior needs.
Request a vision and hearing screening if these have not been don within the last calendar year.
The teacher identifies specific concerns and attempts various instructional strategies or program adaptation to accommodate the student's individual needs. It's import to communicate with eh special education teacher and other staff who might be able to suggest different strategies, modification or accommodation.

SEN Teacher Begins Official SEN Referral
The SEN coordinator will initiate the evaluation and eligibility determination process for approved referrals.
When a referral for evaluation is denied, the SEN coordinator will provide parents with a Prior Written Notice explaining the reasons for not proceeding with an evaluation.
After the decision has been made to proceed with the evaluation, the SEN teacher will provide a Prior Written Notice to the parents indicating a referral for SEN evaluation has been completed and the parents are given a copy of their rights.
A timeline for SEN eligibility determination and placement is established and begins when a signed Consent for Evaluation is obtained. No SEN assessment can begin without parental consent.
The classroom teachers, principal, SEN teacher, Parents and student are key member of the IEP team and their involvement in the process is very important.
