Pt. will need assistance with hygiene care, but may advance to self-care as he is able.

Pt. underwent appendectomy earlier today due to a possible ruptured appendix. Pt.'s peritoneum was inflamed. Pt. was admitted to the medical/surgical unit with NPO status in preparation for procedure. Pt had IV placement and IV fluid 0.9% NaCL at 125 mL/hr. Pt was typed and cross matched for two units of packed red blood cells. An informed consent was signed and the operating room staff was notified. Ampicillin sodium/sulbactam sodium (Unasyn) 1g IV was administered STAT.

My assessment of the situation is the pt. is currently experiencing complications r/t appendectomy for he wants to remain immobile. #

I recommend that the physician see the patient as soon as possible assess need for increased pain medication. #

INTERVENTIONS
Quick pain medication administration; recommend increasing dose if necessary #

Teach deep breathing and relaxation. #
Assess vital signs, I/O's and weight #
Raise head of bed to semi-Fowler's and assess pt.'s RR and lung sounds, O2 sat monitoring
Assess patient's bowel sound s and presence of flatulence. If BM occurs, record time and character. #
Monitor wound incision, drainage, and abdominal distention
Assess level pf pain and administer medication #
