Diagnostic Criteria
DSM 4
- Loss of Memory
- Any 2 of the following
Loss of executive function
Apraxia
Agnosia
Aphasia
- Affecting ability to perform daily activities

Memory Screening
Early Stage Dementia = MOCA
Moderate, Severe Stage = MMSE

History
- Any loss of Short Term Memory
e.g. (forget keys, forget turn off tap)
- Loss of Long Term Memory
- Loss of Executive Function
- Reduction in ability to perform ADLs (Activities of Daily Living)
e.g. cannot cook, cannot go out, cannot go toileting
- Behavioral Changes
- Change in Sleeping Habits
- Signs of Mental Slowing
