Ways they Spread
Water
Some Pathogens are spread by small bacteria in the water, for example Cholera in dirty water.
Air
Some are carried in the air an d can be breathed in, this works through small droplets of water in the air, for example the influenza

Bacterial disease
Two Types
Salmonella, type of bacteria that causes food poisoning, to help prevent it, food is given vaccine of salmonella
Gonorrhea, this is a sexually transmitted disease, it is passes on through unprotected sex, this can be prevented by using protection when having sex
Methods of prevention
Being hygienic, doing things such as washing hands and keeping clean eliminates the bacteria, so it won't spread
Destroying vectors, by getting rid of the things that spread the bacteria, such as insects also stops the spread
Isolating infected people, another way to prevent the spread of disease is to basically just hide from it
Vaccination, vaccinating yourself means that you basically can't catch the disease
